GCM-cipher: just trust the user to call update_ad at the right time

cipher_info->iv_size == 0 is no longer ambiguous, and
cipher_get_iv_size() always returns something useful to generate an IV.

- no support for additional data
- no support for tag

(Only one padding mode recognized yet.)

Fix pattern in tests/.gitignore along the way.

 - Renamed POLARSSL_MODE_CFB128 to POLARSSL_MODE_CFB

 - Added support for NULL cipher (POLARSSL_CIPHER_NULL_CIPHER) and weak ciphersuites (POLARSSL_ENABLE_WEAK_CIPHERSUITES). They are disabled by default!

 - Major type rewrite of int to size_t for most variables and arguments used for buffer lengths and loops

 - Fixed style typos
