Revert changes regarding chat_message_sent callback notification (notified before message is really sent but right after chat_message_send() is called) + fixed crash

no longer issue presence subscription when only_when_registered = true but no proxy config configured.

Fix a crash when a conference established from a list of participants has some of them that decline it, leaving a unique participant.
DO NOT MERGE TO MASTER BRANCH (the new implementation doesn't have the problem).

Fix issue with useless reINVITE to remove from a conference a call that is about to be terminated.
