If all remaining check lists are completed, set the ice session state to completed.

change ms_new calls to ms_new0, as a good practice to avoid facing unitialized values in the future.

The session now contains a table of check lists instead of a list and there can now be gaps between check lists.

This prevents ignoring delayed responses if a new transaction has been
added by a pair.
