The internal on__MenuClosed signal has been renamed
to onAboutToHide in qtquickcontrols. Also update some
import versions while we're at it.
This patch also adds examples OSX application bundles
to .gitignore which only contained the unix binaries.

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.

Refactor JavaScriptDialogManagerQt to support a Qt Quick dialogs
friendly approach.
Qt Quick dialogs are still missing a prompt, so we use a "handmade"
one. This should be solved before 5.3 though.

Import the sources as-is, without adding it to the build, to allow
performing diffs later on the changes that were needed to port it
to use QtWebEngine and manage source compatibility issues.

Cleaner than deciding on startup with an env variable

This layers things properly to be able to implement the UI in the
example application instead of directly in shell_qt.cpp.

This is still using global variables to allow the Shell platform
code to do callbacks to the API classes. This should go away once
we properly implemented a WebContentsDelegate.

We need to replace this with a properly dynamically generated list
of dependencies, but for prototyping this will have to do the trick.
