RTP bundle mode fix: handle the case where a SSRC gets its mid changed (ie the stream is moved to another mid). This can happen during video conference in pure SFU mode, when for instance a stream distributed within the active speaker SDP stream becomes distributed as part of a participant recv-only video stream after a layout change.
